DYNAMIC HEALTH CARE, INC. – NOTICE OF DATA PRIVACY EVENT

Dynamic Health Care, Inc. (“DHC”) is providing notice of a recent event that may affect the privacy of information of certain nursing care facility patients and employees for whom it provides consulting, administrative, and back office services, including Woodbridge Nursing Pavilion, Waterfront Terrace, Bridgeview Health Care Center, Willow Crest Nursing Pavilion, Pavilion of Ottawa, and River North of Bradley Health & Rehabilitation Center. This notification provides information about the event, DHC’s response to it, and resources available to individuals to help protect their information, should they feel it necessary to do so.
What Happened? On November 8, 2020, DHC identified the presence of malware on certain computer systems in its environment. DHC immediately commenced

We are now having indoor and or outdoor visitations with your loved ones. Visitations must be scheduled in advance, only one visit per resident per week, and vistis are open only to adults 18 or older and residing in the same household. Please call and schedule your visits with Christine Simmons 815-431-4900 ext 242
COVID-19 UPDATE
To help all of us get through this we are now scheduling facetime phone conversations via IPhones, or Facebook Messanger for the non IPhone users. The phone conversations will be restricted to 15 minuets. If you are interested in making a facetime phone call please contact Christine Simmons the Activity Director to get placed on the schedule. Calls will be done between the hours of 9am-12pm and start again from1pm-4pm. Monday - Friday. Please try and have one person from the family contact us for a scheduling time so that all residents have the chance to speak to a loved one. Thank you for your understanding
Due to the COVID-19 virus, we are no longer able to accept any items, belongings, or clothing to be brought into the facility for our residents.
In addition, we can no longer allow family members to take laundry out of the facility to be done by family and returned. All laundry will be done by the facility.
We appreciate your cooperation and are sorry for the inconvenience this may cause but we are following the CDC, CMS, and IDPH regulations.
We continue to appreciate your understanding, as we are diligently working to protect the health and safety of our residents and staff during this difficult time.
